CANCELLATION POLICY

BOOKING, FORMS & CANCELLATION POLICY A $100 booking fee is required to reserve your appointment. The booking fee is applied toward your service total and is non-refundable. All consent and medical-history forms must be completed within 24 hours of booking. Your appointment is not fully approved until your forms have been submitted and reviewed. Delaying completion of your forms does not extend the cancellation or rescheduling deadline. You may reschedule one time with at least 96 hours’ notice. Your booking fee will transfer to the new appointment. Appointments cancelled or rescheduled with fewer than 96 hours’ notice will forfeit the $100 booking fee. A new $100 booking fee will be required to reserve another appointment. Appointments booked fewer than 96 hours in advance require immediate completion of all forms. Once the forms have been reviewed and the appointment approved, the booking fee is nontransferable. Same-day cancellations and no-shows will forfeit the booking fee and will also be charged 20% of the service total. Medical conditions, medications, recent procedures, surgery, hospitalization or antibiotic use must be disclosed before the cancellation deadline. If the appointment must be postponed because information was disclosed late, the regular cancellation policy will apply.
